Generally you need to produce the last three years of bank records. Further back than that is likely to be irrelevant UNLESS the timeframe from that period is relevant to the litigation. Yes. You have to produce these records. If you refuse or fail to do so, the other side will bring a motion to compel against you. You will lose and there could be penalties against you.

The bank should be able to produce all of your bank records. There is no six month limit. (One of my clients had to produce the last two years of her bank records.)

All you have to do is to go to the bank and ask for the records. You can do this in person. You might be able to do this on the phone.

If there is a fee, you would generally be expected to pay that fee. If you genuinely cannot afford to pay the fee, you can send a letter to the opposing party that explains how much it will cost and ask them to pay this amount.

You have to make a reasonably attempt to gain the documents. If you can call the bank and can obtain the documents, you can not state you simply didn't know that. If the bank charges a unreasonably fee, then you can state you cannot produce the documents because you cannot produce the cost of the documents. You can also state that the request is vague and ambiguous, over broad and unintelligible and you don't have 5 years worth of bank statements.
